About Us Requirements Of Documents Related to Passport Services Offered to Indian Nationals

1. Any change in laminated pages of passport would require to apply for a fresh passport booklet.

2. If an applicant is married, he/she has to include his/her Spouse name in passport, for that Marriage Registration Certificate is must. Concealment of marriage is an offence.

3. For change of name in the passport, applicant has to provide either

a) Newspaper clippings for change of name in the passport

OR

b) Name change Gazette notification published in the Union/State Official Gazette

Re-Issuance of Passport on Expiry of Its Validity (If Validity of Passport Is Expiring)

i) Applicant can apply for re-issue of passport when its validity is expiry within one year.

ii) Application form for re-issue of passport can be downloaded from our website:http://passport.gov.in

iii) Self attested copies of passport's first two and last two pages including all the pages of observation (if any), made by the Passport Issuing Authority.

iv) A copy of passport page containing visa/work permit/permanent residence status;

v) Three passport sized photographs (coloured; 5cm x 5cm with white background)

Re-Issuance of Passport If All the Pages Have Exhausted

iii) A copy of passport page containing visa/work permit/permanent residence status;

iv) Copy of last six pages of the passport;

v) Three passport sized photographs (coloured);

Re-Issue of Fresh Passport Booklet on Extension of Short Validity Passport

Re-Issuance of Passport In Lieu of Damaged Passport

i) Application form for duplicate passport in lieu of damaged passport can be downloaded from our website:http://passport.gov.in

ii) Self-attested copies of passport's first two and last two pages including all the pages of observation (if any), made by the Passport Issuing Authority.

iii) A copy of passport page containing visa/work permit/permanent residence status;

iv) Three passport sized photographs (coloured);

v) Declaration stating how and where the Passport got damaged (Annexure "F");

Re-Issuance of Passport In Lieu of Lost/Stolen Passport

i) Application form for lost/stolen passport can be downloaded from our website:http://passport.gov.in

ii) Self attested copies of passport's first two and last two pages including all the pages of observation (if any), made by the Passport Issuing Authority.

iii) A copy of passport page containing visa/work permit/permanent residence status;

iv) Three passport sized photographs (coloured);

v) Proof of present residence;

vi) Declaration stating how and where the Passport got lost or stolen (Annexure "F");

vii) Copy of police report stating the circumstances in which passport is lost or stolen;

viii) A letter from the employer on the letter head of the organisation stating that the applicant was employed with them and duration of his/her employment. Applicant will also procure a copy of visa/work permit from the employer (as it is a mandatory requirement for the employer to keep a copy of the passport/visa at the time of appointment of the employee);

Issuance of an Emergency Certificate (EC)

i) Application form for issuance of Emergency Certificate (EC) can be downloaded from our website:http://passport.gov.in

ii) Self attested copies of passport's first two and last two pages including all the pages of observation (if any), made by the Passport Issuing Authority.

iii) A copy of passport page containing visa/work permit/permanent residence status;

iv) Four passport sized photographs (coloured);

v) Declaration stating how and where the Passport got lost or stolen/damaged (Annexure "F");

vi) Copy of police report stating the circumstances in which passport is lost (in case of lost passport);

vii) A copy of valid air-ticket to return to India;

ix) Proof of Residential address in South Africa/Lesotho;

x) Written application mentioning the reason for applying an EC;

xi) Any other document as specified by the Consular Officer;

It may be noted that issue of Emergency Certificate is intended for one way travel to India only and results in cancellation of existing passport of the holder. On arrival in India, he/she will have to contact the concerned Passport Issuing Authority in India for issue of new passport.

Issuance of Passport to a Minor Child Born In South Africa/Lesotho.

i) Application form for passport for children born in South Africa can be downloaded from our website:http://passport.gov.in

ii) Self-attested copies of the parent’s passports first two pages and last two pages;

iii) A copy of passport page containing visa/work permit/permanent residence status of both the parents;

iv) Three passport sized photographs (coloured);

v) Proof of present residence;

vi) Both parents should sign on the form at the prescribed column under declaration by parents/guardians in case of a minor. In case of infants thumb impression should be affixed within the box on Page No. 1 and Page No. 2 after Serial No. 26 of the form.

vii) Copy of Birth registration Certificate issued the Indian High Commission/Consulate and birth certificate issued by South African Home Department and local hospital in case of South African/Lesotho born babies.

ix) Proof of Residential address in South Africa;

x) Five-years validity passport is issued;

Attestation of an Affidavit by the Parent Residing Abroad Giving No Objection Certificate (NOC) For Issuance of Passport to a Minor Child in India

i) Application form can be downloaded from our website:http://passport.gov.in(Opt for Miscellaneous Services in the form. Application form is same as re-issue of passport)

ii) A copy of the self-attested parent/s (residing abroad/South Africa/Lesotho) passports first two and last pages, which contains personal particulars of the passport holder;

iii) A copy of passport page containing visa/work permit/permanent residence status of the parent/s;

iv) One passport sized photographs to be pasted on the application and a photo each on the original Affidavit (coloured) and its photocopy;

v) Proof of present residence;

vi) Affidavit as per specimen enclosed (in duplicate).

vii) A fee of Rand 330/- (including ICWF Fee of Rand 30.00).

Re-Issuance of Fresh Passport Booklet (Change of Name/Damaged/Lost/) Stolen, etc.

The applicant has to take following action to apply for re-issue of fresh booklet due to complete change of name on the passport:

i) An application form is available on our website:http://passport.gov.in

ii) (Form is same as for passport renewal and applicant can opt for Miscellaneous Service re-issue of fresh booklet due to change of name).

iii) A copy of visa/work permits of the applicant.

iv) Sworn Affidavit.

v) Original newspaper cuttings announcing the change of name published in two leading daily newspapers (one popular daily newspaper in the area of the applicant's present address and another at his permanent address).

Re-Issue of Passport for Change in Surname after Marriage

The applicant has to take following action to apply for re-issue of fresh passport booklet due to change in surname (woman applicant) after marriage:

iii) A certified copy of visa/work permits of the applicant and her spouse.

iv) Self-attested copies of the spouse's passport.

v) Attested copy of copy of marriage certificate by the State Government concerned as well as by MEA, Patiala House, New Delhi or an Affidavit (jointly signed).
