BIRTH CERTIFICATE

How to Apply: Download the Miscellaneous Services Form and submit along with:

  •  Self attested copy of passport (first page and last page);

  •  Self attested copy of visa/work permit/residence permit;

  •  Proof of Residential address in South Africa. This can be a copy of any one of the following:

    •      South African Driving license.

    •      Water or landline telephone bill displaying applicant’s address

    •      House Lease Agreement

    •      State Identification Card

    •      Note: Bank/credit card/mobile phone statements are not accepted as residence proof

Note :Please carry originals of all the supporting documents to be presented to the Consular Officer (photocopies of which are to be attached with the application).
