How to Apply: Download the “Miscellaneous Service Form” and submit along with:

⦿ Self-attested copy of passport (first page and last page);

⦿ Self-attested copy of visa/work permit/residence permit;

⦿ For marriage certificate, the name of the spouse must be entered in the Passport of the applicant.

⦿ Letter explaining the reason for Marriage certificate

⦿ Proof of Residential address in South Africa/Lesotho.

⦿ Any other document as specified by the Consular Officer

Note: Please carry originals of all the supporting documents to be presented to the Consular Officer (photocopies of which are to be attached with the application).
